INTRODUCCIÓN: La tomosíntesis o mamografía digital 3D es una nueva modalidad que ha demostrado, en diversos estudios europeos, mejorar los resultados de la mamografía digital 2D en el cribado mamario. Sin embargo, son necesarios más estudios en este contexto, siendo desconocido el efecto de la tomosíntesis en los programas de cribado mamario de nuestro país. OBJETIVOS: Comparar la tomosíntesis frente a la mamografía digital 2D en términos de tasas de detección de cáncer y de rellamadas. Evaluar la lectura única de tomosíntesis más mamografía sintetizada como nueva estrategia de trabajo en el cribado mamario. Valorar si la mamografía sintetizada evita la realización de la mamografía digital 2D. POBLACIÓN, DISEÑO Y METODOLOGÍA: se incluyeron mujeres (50-69 años) de nuestra área hospitalaria, citadas en el cribado mamario y que aceptaron participar mediante la firma del consentimiento informado, realizándose mamografía digital 2D y tomosíntesis. Se obtuvo mamografía sintetizada a partir de las imágenes de la tomosintesis. Se evaluaron cuatro modelos de lectura independientes: a) Mamografía digital 2D-1ª lectura; b) Mamografía digital 2D-2ª lectura; c) Tomosíntesis más mamografía sintetizada-3ª lectura; d) Tomosíntesis más sintetizada más digital 2D-4ª lectura. Se consideró doble lectura de mamografía la combinación de la 1ª y 2ª lecturas, y doble lectura de tomosíntesis la combinación de la 3ª y 4ª lecturas. La comparación de los cánceres detectados y rellamadas entre las técnicas se realizó mediante la prueba estadística McNemar para datos apareados, considerándose significativo un valor p < 0,005. RESULTADOS: En 16.067 participantes se detectaron 98 cánceres y 1.196 fueron rellamadas. Al comparar las dobles lecturas de ambas técnicas se identificó un aumento significativo del 17´4% (p= 0,004) en la detección de cáncer y una reducción del 12´5% (p= 0,001) de rellamadas con la tomosíntesis. La lectura única de tomosíntesis más sintetizada supuso un aumento significativo en la detección de cáncer del 12´6% (p= 0,043) y una reducción en las rellamadas del 40´5% (p< 0,001) frente a la doble lectura de mamografía digital 2D. La mamografía sintetizada no fue inferior a la mamografía digital 2D al no identificar diferencias significativas en su comparación. CONCLUSIONES: La incorporación de la tomosíntesis ha demostrado un aumento significativo de la detección de cáncer y una reducción de rellamadas al comparar las dobles lecturas de ambas técnicas. La lectura única de tomosíntesis más mamografía sintetizada ha sido superior a la doble lectura de mamografía digital 2D en términos de detección de cáncer y rellamadas. La no inferioridad diagnóstica obtenida por la mamografía sintetizada permite evitar la realización de la mamografía digital 2D.; INTRODUCTION: Tomosynthesis or 3D-mammography is a new equipment that has been shown to improve the results of 2D-mammography in breast screening-programme in various European studies. However, more studies are needed in this context as the effect of tomosynthesis in breast screening-programme in our country being is unknown. OBJECTIVE: To compare tomosynthesis versus 2D-mammography in terms of cancer detection rate and recalls rate. To evaluate the single reading of tomosynthesis plus synthesized mammography as a new screening strategy. To assess whether synthesized mammography avoid performing 2D-mammography. MATERIAL AND METHODS: Women (age 50-69 years) from our hospital area, appointed in breast screening-programme and who agreed to participate by signing informed consent, performing 2D-mammography and tomosynthesis were included. Synthesized mammography from the tomosynthesis images was obtained. Four independent reading models were evaluated: a) 2D-mammography (1st reading); b) 2Dmammography (2nd reading); c) tomosynthesis plus synthesized mammography (3rd
reading); d) tomosynthesis plus synthesized plus 2D-mammography (4th reading). The combination of the 1st and 2nd readings was considered double reading of 2Dmammography, and the combination of the 3rd and 4th readings was considered double reading of tomosynthesis. The comparison in cancer detected and recalls between techniques was performed using the McNemar´s test for paired data, considering a value of p< 0,005. RESULTS: Out of 16067 participants, there were 98 cancers and 1196 recalls. Having compared double reading of 2D-mammography with double reading of tomosynthesis, there was a significant increase in cancer detection of 17.4% (p= 0,004) and a decrease in recalls of 12.5% (p= 0,001). The single reading of tomosynthesis plus synthesized meant a significant increase in cancer detection of 12.6% (p= 0,043) and a decrease in recalls of 40.5% (p< 0,001) compared to the double reading of 2D-mammography. Synthesized mammography was not inferior to 2D-mammography because it did not identify significant differences in its comparison. CONCLUSIONS: The incorporation of tomosynthesis has demonstrated a significant increase in cancer detection and a decrease in recalls compared when comparing the double readings of both techniques. The single reading of tomosynthesis plus synthesized has been superior to the double reading of 2D-mammography in terms of cancer detection and recalls. The diagnostic non-inferiority obtained by synthesized mammography makes it possible to avoid performing 2D-mammography.

This study reveals the existence of oxidative stress (reactive oxygen species (ROS)) in non-nervous organs and tissues in multiple sclerosis (MS) by means of a model of experimental autoimmune encephalomyelitis (EAE) in rats. This model reproduces a similar situation to MS, as well as its relationship with intestinal microbiota starting from the changes in bacterial lipopolysaccharide levels (LPS) in the outer wall of the gram-negative bacteria. Finally, the administration of extra-virgin olive oil (EVOO), hydroxytirosol (HT), and oleic acid (OA) exert beneficial effects. Twenty-five Dark Agouti two-month-old male rats, weighing around 190 g, were distributed into the following groups: Control, EAE (experimental autoimmune encephalomyelitis group), EAE + EVOO, EAE + HT, and EAE + OA. The glutathione redox system with the EAE was measured in heart, kidney, liver, and small and large intestines. The LPS and the correlation with oxidative stress in the small and large intestines were also investigated. The results showed that (1) the oxidative damage in the EAE model affects non-nervous organs and tissues; (2) The LPS is related to inflammatory phenomena and oxidative stress in the intestinal tissue and in other organs; (3) The administration of EVOO, HT, and OA reduces the LPS levels at the same time as minimizing the oxidative damage; (4) EVOO, HT, and OA improve the disease’s clinical score; and (5) on balance, EVOO offers a better neuroprotective effect.

El presente proyecto surge para dar respuesta a una serie de problemas que se dan durante el proceso de elaboración y defensa de los Trabajos Fin de Grado en el marco de la Universidad de Córdoba. A pesar de que actualmente existen esfuerzos por mejorar los criterios aplicados en los distintos centros de la UCO, se ha podido comprobar una falta de uniformidad en las guías y en las recomendaciones ligadas a la realización de los TFG, algo que, salvando la comprensible adecuación a las características de cada área, puede suponer un riesgo para la consecución de un sistema de evaluación justo. Además, algunas facultades carecen de criterios públicos de evaluación, lo que puede dar lugar a situaciones arbitrarias o irregulares. Para ello, en primer lugar, se ha analizado la opinión de profesorado y alumnado con respecto al TFG. Con el fin de resolver las deficiencias encontradas, se ha propuesto una solución que englobe todos los centros que forman la UCO: una herramienta que contemple las competencias básicas reconocidas por la legislación actual, comunes a todos los grados, y que tengan naturaleza transdisciplinar, para, a partir de ahí, poder construir un sistema de evaluación más razonable, coherente y uniforme para toda la Universidad de Córdoba.; The present project mains to solve a series of problems that occur during the process of preparation and defence of the undergraduate final dissertation (TFG) within the University of Córdoba. Although there are currently efforts to improve the criteria applied in the different centres of the UCO, it has been possible to verify a lack of uniformity in the guides and in the recommendations related to the realization of the TFG, something that, beyond the understandable adaptation to the characteristics of each area, may pose a risk to the achievement of a fair evaluation system. In addition, some faculties lack public evaluation criteria, which may give rise to arbitrary or irregular situations. To do this, first of all, the opinion of teachers and students regarding the TFG has been analysed. In order to solve the deficiencies found, a solution has been proposed that encompasses all the centres that make up the UCO: a scale that contemplates the basic competences recognized by the current legislation, common to all grades, and of a transdisciplinary nature to be able to build a more reasonable, coherent and uniform evaluation system for the entire University of Córdoba.

Introducción - Las herramientas educativas en línea permiten una formación médica
interactiva para estudiantes dispersos geográficamente. Pocos estudios tienen como objetivo
primario su validación objetiva. Por sus ventajas logísticas, se utilizó un taller de delineación
en línea (TDE) para homogeneizar el contorneo como control de calidad de radioterapia en
el ensayo prospectivo multicéntrico europeo RAIDs (Rational molecular Assessments and
Innovative Drug Selection) en cáncer de cérvix localmente avanzado (CCLA). Se realizaron
dos TDE idénticos para evaluar sobre un paciente de simulacro las delineaciones de los
centros RAIDs.
Objetivos
.- Principales:
1. Realizar una validación interobservadora de los TDE en CCLA mediante una evaluaci
ón cualitativa y cuantitativa de la mejoría de delineaciones entre participantes
(variabilidad interobservadora).
2. Efectuar una validación intraobservadora del TDE en CCLA mediante una evaluaci
ón cualitativa y cuantitativa de la mejoría de delineaciones de cada participante
(variabilidad intraobservadora).
.- Secundarios:
1. Evaluar la metodología de enseñanza del TDE mediante cuestionarios de satisfacci
ón.
2. Análizar las delineaciones de los participantes de los centros europeos que utilizan
la resonancia magnética para planificar braquiterapia (BQT) con respecto a los
que no la usan.
Metodología - Se realizaron dos TDE incluyendo 46 especialistas de 14 centros de
RAIDs. Se estableció una colaboración técnica con la Sociedad Europea de Oncología Radioter
ápica (ESTRO). La formación se realizó por una experta en la materia, CHM. Mediante
3 presentaciones en directo en línea se presentaron la plataforma de contorneo Fellowship in Anatomical deLineation and CONtouring (FALCON) EduCaseTM, las guías de delineaci
ón y los contornos de los participantes. Los participantes completaron contornos basales
(C1), guía (C2) y finales (C3) para radioterapia externa (RTE) y braquiterapia (BQT)
en CCLA. La variabilidad interobservadora e intraobservadora se evaluó cuantitativamente
(Índice DICE) y se analizaron mediante un modelo lineal mixto. La variabilidad intraobservadora
fue evaluada cualitativamente mediante el test de McNemar.
Resultados - Nueve participantes enviaron contornos para RTE y BQT (C1-C3). Treinta
y dos envió algún contorno. La comparación interobservadora cuantitativa de RTE mostró
una mejoría significativa entre C2 y C1 para intestino, CTV ganglionar, CTV-p y GTVganglio
con un detrimento significativo para GTV-ganglio (entre C3 y C1; C2), CTV-p
(entre C3 y C2) e intestino (entre C3 y C2), es decir, una mejoría en general entre C2 y
C1, con un detrimento significativo entre C3 y C2 en dos volúmenes blanco y un órgano
de riesgo. Para BQT hubo una mejoría significativa entre C2 y C1 para vejiga, GTV, HRCTV
e IR-CTV con un detrimento significativo para vejiga (entre C3 y C2), en resumen,
una mejoría general entre C2 y C1, con sólo un detrimento entre C3 y C2 para la vejiga.
Las comparaciones intraobservadoras cuantitativas mostraron una mejoría signi cativa de
delineaciones de regiones de interés entre C2 y C1, C3 y C1 y C3 y C2 para RTE y entre
C2 y C1 para BQT. Las comparaciones cualitativas intraobservadoras destacables en BQT
fueron una mejoría significativa en las direcciones derecha y posterolateral derecha para los
volúmenes blanco entre C2 y C1, que pasaron a ser signi cativamente peores para estas
direcciones entre C3 y C1; C2. El resultado de las preguntas acerca de la organización y
el contenido del TDE del cuestionario de satisfacción (escala del 1 al 5, 1=poco satisfecho,
5=muy satisfecho) para los 20 participantes que respondieron fue de media 4.36 (rango:
3.95-4.60). Los centros que empleaban resonancia magnética para la delineación en BQT
tuvieron una mejoría significativa para HR-CTV con respecto a los que usaban otras técnicas
(entre C2 y C1: p-value < 0.005; entre C3 y C1: p-value = 0.02).
Conclusiones - Los TDE permiten formar, armonizar inicialmente la delineación y evaluar
la experiencia de los centros antes de la inclusión de pacientes de una forma cómoda y
eficaz en el seno de un estudio multicéntrico.; Introduction - E-learning programmes allow e ective medical training for geographically
dispersed participants. Few studies aim primarily to validate these programmes objectively.
Considering its logistical aptitudes, an ODW using FALCON was used to homogenise
delineation practice in cervical cancer as quality control among European centres participating
in a multicentre prospective trial on locally advanced cervical cancer (LACC). Two
identical ODW were performed, to evaluate the delineations of RAIDs centres on a ctitious
patient.
Aims
Primary endpoints:
1. Interobserver validation of the ODW in LACC by using qualitative and quantitative
assessments of the improvement of clinician contours between clinicians
(interobserver variability).
2. Intraobserver validation of the ODW in LACC by assessing qualitatively and
quantitatively the improvement of the clinician contours for each clinician (intraobserver
variability).
Secondary endpoints:
1. Evaluation of the teaching methodology of the ODW as re ected by the participant
satisfaction questionnaires.
2. Analysis of clinician contouring on magnetic resonance imaging (MRI) for European
centres which use MRI for brachytherapy planning versus those which do
not.
Methods - Two ODW included 46 clinicians from 14 RAIDs centres. A technical collaboration
was established with European Society for Radiotherapy & Oncology (ESTRO).
Training was performed by an expert in the eld, CHM. Through 3 live online presentations, the contouring platform FALCON EduCaseTM, the delineation guidelines and the participant
contours were presented. Participants were asked to complete baseline contouring
(C1), guideline contouring (C2) and nal contouring (C3) for external beam radiotherapy
(EBRT) and brachytherapy (BT) for LACC. Interobserver and intraobserver variability was
quantitatively evaluated (DICE index) and analysed by a linear mixed model. Intraobserver
variability was qualitatively evaluated by the McNemar test.
Results - Nine participants contoured for EBRT and BT for C1 - C3. Thirty-two
clinicians submitted any contour.
Concerning interobserver quantitative comparisons for EBRT, signi cant improvement
was observed for C2 vs. C1 for bowel, nodal elective volume (CTV-node), GTV-p, uterus and
vagina, at least 20 mm below GTV-p (CTV-p) and radiologically pathological lymph nodes
to boost (GTV-node), versus a signi cant detriment for GTV-node (C3 vs. C1; C2), CTV-p
(C3 vs. C2) and bowel (C3 vs. C2), showing overall an improvement in C2 vs. C1, versus a
detriment in C3 vs. C2 for two target volumes and one organ at risk. In the BT treatment
there was signi cant improvement for C2 vs. C1 for bladder, gross tumor volume (GTV), high
risk CTV (HR-CTV) and intermediate risk CTV (IR-CTV), versus a signi cant detriment for
bladder (C3 vs. C2), thus a general improvement in C2 vs. C1, with only a detriment in C3
vs. C2 for bladder. As for intraobserver quantitative comparisons, a signi cant improvement
was observed for contouring a region of interest between C2 vs. C1, C3 vs. C1 and C3 vs. C2
for EBRT and between C2 vs. C1 for BT. Notable intraobserver qualitative comparisons were
found for BT, a signi cant improvement towards the right and posterolateral right directions
for target volumes in C2 vs. C1, which became signi cantly worse in these directions in C3
vs. C1 and C2. The average result of the Organisation and Content items in the satisfaction
questionnaire for the 20 ODW participants who responded (scale 1-5, 1=poor, 5=excellent)
was 4.36 (range 3.95-4.60). With regard to the imaging technique used for BT planning,
centres using magnetic resonance imaging (MRI) did significantly better in the BT case for
HR-CTV than centres using other techniques (C2 vs. C1: p-value < 0.005; C3 vs. C1:
p-value = 0.02).
Conclusions - ODW allow to train, initially harmonise contouring and assess the experience
of centres before patient inclusion conveniently and eficiently in the context of a
multicentre trial.
